Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join our established primary care team at
Primary Care Hampton in Hampton, Virginia.

Position ResponsibilitiesThe Advanced Practice Provider will:
- Perform comprehensive patient assessments and physical examinations
- Diagnose and manage acute and chronic medical conditions across the lifespan
- Assess patient health status and develop evidence-based treatment plans
- Order, perform, and interpret diagnostic studies and laboratory testing
- Prescribe medications, treatments, and therapies in accordance with Virginia regulations and Riverside policies
- Counsel and educate patients and families regarding diagnoses, treatment options, preventive care, and disease management
- Review and analyze diagnostic results and adjust treatment plans as appropriate
- Manage wellness visits, preventive screenings, and follow-up care
- Collaborate with physicians, specialists, and interdisciplinary team members to optimize patient outcomes
- Accurately document patient encounters and clinical findings within the electronic medical record
Education RequirementsNurse Practitioner- Master's Degree in Nursing from an accredited program
- Current Virginia Nurse Practitioner license
- Current Nurse Practitioner certification
Physician Assistant- Master's Degree from an accredited Physician Assistant program
- Current Virginia Physician Assistant license
Licensure and Certification RequirementsNurse Practitioner- Active Registered Nurse license in Virginia
- Active Virginia Nurse Practitioner license with appropriate specialty designation
- National certification in applicable specialty:
- Family Nurse Practitioner (FNP-BC or FNP-C)
Physician Assistant- Active, unrestricted Virginia Physician Assistant license
- Current NCCPA certification
Prescriptive Authority Requirements- Active DEA registration or eligibility to obtain registration
- Eligibility for Schedule II-VI prescriptive authority in the Commonwealth of Virginia
Qualifications- Ability to make independent clinical decisions regarding patient care
- Experience diagnosing and treating common acute and chronic medical conditions
- Working knowledge of managed care and Medicare populations
- Strong interpersonal and communication skills
- Professional demeanor and appearance
- Basic computer proficiency
