What You'll Do Position SummaryThe Neighborhood Center Advanced Practice Provider serves as a health-care professional who drives, supports, diagnose, and cares for a variety of health-related problems for Suvida Healthcare's patients. The provider will manage these patients with the supervision of the Neighborhood Center Medical Director in a multidisciplinary value-based care model. The provider work assignments involve low-emerging risk complex patients to high complexity patients where the analysis of situations or data requires an in-depth evaluation of variable factors. The clinician will collaborate with a multidisciplinary care team and will serve as the leader of their respective care team within their Neighborhood Center. Essential responsibilities consist of but are not all inclusive:
Responsibilities- Cares for Suvida Healthcare's patients and focuses on outpatient medicine, continuity of care, health maintenance, improving health outcomes, and disease prevention
- Manages patients with the supervision of the Neighborhood Center Medical Director in a multidisciplinary value-based care model
- Serves as the leader for their respective care team within the Neighborhood Center
- Collaborates effectively with Suvida Healthcare's multidisciplinary care team
- Focuses on providing 5-Star care and strives to provide a high level of customer service
- Keeps a medical history and medical records
- Refers the patient to specialists as needed
- Attends daily morning huddle with care team to discuss patients to be seen that day and works to close any potential care gaps
- Attends weekly huddle with multidisciplinary care team to review and manage high-risk patients within the care team
- Begins to influence department's strategy
- Makes decisions on moderately complex to complex issues regarding technical approach for project components, and work is performed without direction
- Exercises considerable latitude in determining objectives and approaches to assignments
What You'll BringKnowledge, Skills, and Abilities- 2+ years minimum experience, preferably in geriatrics
- Active, unrestricted medical license in Texas (for Tx. Centers) or Arizona (for Az. Centers)
- Current on all required immunizations
- A working understanding and support of a managed care, full risk, value-based care model preferred
- Strong leadership, customer service, and spoken/written communication skills
- Must have the ability to demonstrate self-confidence and positive attitude toward self and others
- Excellent team player and communication skills required
- Proficiency in Microsoft office platforms including word, excel, and power point
- Bilingual/Bicultural (English and Spanish) Preferred
Education, Experience, Licensure, or Certification Requirements- Nurse Practitioner (NP) or Physician Assistant
- Board certification or eligibility in Family, Adult, and Geriatrics
- Current DEA license
