Responsibilities- Monday - Friday, 8am-5pm
- Rotating and flexible on-call schedule.
- We use eClinical Works (eCW), along with partnering programs to ensure ease of use for our patients and practices.
- We focus on continuity of care, health maintenance, and disease prevention, working alongside patients and their families to prevent, diagnose, treat and manage a population with multiple comorbidities.
- Integrated team-based care: Working as a multidisciplinary team to improve patient outcomes by meeting their needs and preferences.
- Prioritizing preventative care and proactive management of chronic conditions to improve patient outcomes, decrease patient's costs, and increase revenue for the practice.
- Providers must be comfortable managing a primarily adult-aged patient population.
Qualifications- Certification through the American Academy of Nurse Practitioners (AANP) OR American Nurses Credentialing Center (ANCC)
- Certification through the American Academy of Nurse Practitioners (AANP) OR American Nurses Credentialing Center (ANCC) program, focusing on adult medicine
- Certification through an accredited PA certification program focusing on adult medicine
- Currently licensed as an Acute, Adult/Gerontological or Family Nurse Practitioner in the state of practice or the ability to obtain a license prior to employment
- Ideal Candidate, with a minimum of two years of experience as Nurse Practitioner
- Bilingual English Spanish speaking preferred
- Holds or is eligible for DEA licensure and CDS licensure (if required in the state of practice)
- Up-to-date CPR Certification and Basic Cardiac Life Support certification
Required Skills- Strong organizational and prioritization skills
- Ability to use Electronic Medical Record (EMR) system
- 2 years of experience of Medical Advantage, HEIDS, MRA and ICD-10 Coding
- Comfortable managing a primarily adult-aged patient population
- Demonstrated ability to problem-solve complex and multifaceted situations
Preferred Skills- Bilingual (English-Spanish) Preferred

Pay Range: $115,000 - $125,000 per hour
