ACO Director

Job Description

The Accountable Care Organization (“ACO”) Director provides direct oversight of Sarah Bush Lincoln Health Center and Fayette County Hospital’s ACO and CMS’ Medicare Share Savings Program (MSSP). The ACO Director will be accountable for on-going operations of the ACO and will develop processes to assist our organizations in meeting quality and financial targets for the MSSP. The Director will provide strategic and operational direction to enhance the value of services provided to the patients, hospitals, physicians and payors in the market, while improving spend efficiencies. This position will also have oversight of our Care Coordination Department.

Hours: Full-time/40 hours per week

Responsibilities

Articulates and implements a quality improvement process that emphasizes teamwork, measurement and accountability., Articulates and implements a quality improvement process that emphasizes teamwork, measurement and accountability., Collaborates with the Medical Director to develop, oversee and monitor quality improvement in Alternative Payment Model (“APM”) mandated quality measures, measurement strategy and by creating tools to support projects, as appropriate., Coordinates discussions with IT system vendors to effectively manage data analytics to support the ACO., Demonstrates and communicates the connection between quality improvement and clinical integration., Determines funds and budget for operations including constructing models for financing operations., Develops and implements ACO strategies to improve health outcomes, the patient experience, and healthcare value including risk stratification methods, data management and tracking, case management across the care continuum, development of care models for post-acute facilities, and feedback to providers on outcomes and utilization, Develops policies and procedures necessary for the success of an ACO program., Identifies successful APM models and programs nationally that can be applied to the ACO., Leader-Coach:
