Qualifications

• Active book of healthcare business within the territory required
• Healthcare sales experience preferred in home health, SNF, DME, hospice, or clinical liaison
• Clinical background (RN, LPN, MSW, etc.) preferred
• Strong communication, presentation, and relationship-building skills
• Detail-oriented with strong organizational skills
• Ability to travel within the territory
• Valid driver's license and reliable transportation required

Responsibilities

• Build and maintain strong referral relationships
• Drive census growth by meeting or exceeding sales goals
• Identify resident care needs and coordinate appropriate services
• Act as a resident advocate to ensure smooth transitions to home care
• Manage the referral-to-admission process efficiently
• Collaborate with physicians, nurses, and interdisciplinary teams
• Provide education to residents, caregivers, and community partners
