ROLE

The Clinical Integration Associate plays a key role in advancing the development and implementation of the Primary Care Network (PCN) within the Huron Perth and Area Ontario Health Team (HPA-OHT). This position supports both the administrative coordination and clinical integration activities required to strengthen collaboration across primary care, health system partners, and community-based services.

REQUIRED EDUCATION & CERTIFICATION(S)

• University degree in public, business, or healthcare administration
• Change Management / Project management qualification (PP, CAP, Project+) or equivalent work experience (preferably healthcare and/or population health related)

REQUIRED SKILLS & EXPERIENCE(S)

• Minimum three years of experience in healthcare, primary care, health system improvement, population health, or a related field.
• Experience supporting project management, quality improvement, or system transformation initiatives.
• Experience coordinating stakeholders across multiple organizations, sectors, or disciplines.
• Experience supporting the development, implementation, or evaluation of integrated care models, clinical pathways, or care coordination initiatives.
• Knowledge of Ontario Health Teams, Primary Care Networks, Ontario Health priorities, and health system integration principles.
• Experience working with clinicians and healthcare providers to advance practice improvement and service integration.
• Experience preparing reports, presentations, briefing materials, meeting documentation, and performance monitoring tools.
• Familiarity with change management methodologies and supporting organizational or system-level change initiatives.
• Experience using data, performance indicators, and evidence to inform planning, decision-making, and quality improvement activities.
• Strong understanding of primary care, community-based services, and the Ontario healthcare system.
• Ability to coordinate multiple projects, priorities, and timelines in a fast-paced and evolving environment.
• Demonstrated ability to build collaborative relationships with clinicians, health system partners, community organizations, and other stakeholders.
• Strong facilitation, communication, and stakeholder engagement skills, both written and verbal.
• Ability to support the development, implementation, and evaluation of clinical pathways and integrated models of care.
• Strong analytical and problem-solving skills, with the ability to interpret data and translate findings into actionable recommendations.
• Proficiency in project management principles, meeting coordination, documentation development, and performance tracking.
• Ability to work independently while contributing effectively as part of interdisciplinary and cross-sector teams.
• Demonstrated organizational skills with attention to detail and the ability to establish structure within ambiguous environments.
• Proficiency in Microsoft Office applications, virtual collaboration platforms, and healthcare-related digital tools.

DESIRED SKILLS, EXPERIENCE(S) & EDUCATION

• Knowledge of digital health tools, eReferral, care coordination technologies, and integrated care enablers is considered an asset.
