Salary: $73,000 - $100,000

Wound Care Coordinator (RN/LPN)

The Wound Care Coordinator oversees assessment, planning, and treatment for residents with wounds or skin integrity concerns. This role also leads the facility's Weight Management Program, ensuring compliance with regulations and delivering high-quality, resident-centered care.

Key Responsibilities

Assess, treat, and monitor wounds and skin conditions; create and update care plans
Complete wound documentation within 48 hours and conduct weekly wound rounds
Track healing progress and adjust treatments based on best practices
Ensure accurate staging and documentation per NPUAP guidelines
Collaborate with providers, families, and the interdisciplinary team
Participate in MDS (Section M) and care planning processes
Educate and train nursing staff on wound care practices
Support discharge planning for residents with wound care needs
Track treatments and support accurate billing

Weight Management

Monitor resident weight trends and identify significant changes
Implement interventions for weight loss/gain with care team
Maintain documentation, care plans, and QAPI reporting

Leadership & Support

Participate in QA meetings and report outcomes/trends
Support nursing leadership duties (MOD rotation as needed)
Assist with policy updates, budgeting, and staff training
Provide hands-on care and treatments within scope
Promote a person-centered care environment

Qualifications

Active Illinois RN or LPN license

Wound care certification preferred
1+ year wound care experience (long-term care preferred)
