Qualifications

• Active RN license required; MPT/DPT with leadership approval considered.
• Completion of Skin Health Team Lead orientation and Wound Basics within 90 days of hire is mandatory.
• National accredited wound certification or commitment to obtain within one year of hire is needed.
• Knowledge of clinical charting and incident reporting is essential.
• Strong understanding of healthcare standards and nursing home regulations is required.
• Experience in team building and respecting interprofessional team dynamics in wound management is essential.
• Proficient in Microsoft Office applications including Word, Excel, and PowerPoint.

Responsibilities

• Lead and integrate a skin health and wound management program with the nursing center's management.
• Serve as the clinical expert in skin, wound, and incontinent products for central supply.
• Oversee durable medical equipment for wound prevention and management.
• Ensure compliance with national wound guidelines and standards of care.
• Coordinate and participate in monthly support calls for ongoing training and development.
• Provide training on wound management to nursing staff and interprofessional team members.
• Monitor and track wound care outcomes and implement necessary improvements.
