Qualifications
- Graduate of an approved, accredited school of professional nursing required; BSN preferred - Current licensure to practice as a Registered Nurse in the Commonwealth of Massachusetts required - Current CPR certification required - Minimum of one (1) year medical/surgical nursing experience required; hospice/home care experience preferred - Demonstrated excellent verbal and written communication skills required - Demonstrated strong interpersonal, organizational, and time management skills required - Demonstrated knowledge and understanding of the dying process with an ability to identify and treat signs and symptoms of the same including competency in pain and symptom management required - Current driver's license, a good driving record, and reliable transportation required.

The Hospice RN Case Manager provides skilled care and case management to clients/families in accordance with the State Nurse Practice Act and accepted professional hospice standards of practice. The Hospice RN Case Manager provides services utilizing nursing theory and processes, communicates/documents observations and assessments, and promotes and maintains quality of life for clients/caregivers through direct care, teaching, and counseling in accordance with the interdisciplinary team care plan.
