Position Overview

The Pharmacist plays a critical role in supporting safe, effective, and evidence-based medication therapy management for members. This role collaborates with physicians, nurse practitioners, care coordinators, and interdisciplinary care teams to ensure appropriate medication utilization, adherence, and optimization of therapeutic outcomes.

The Pharmacist conducts medication reviews, identifies potential drug interactions, supports formulary compliance, and provides clinical recommendations that improve patient safety and health outcomes. This role contributes to population health initiatives by promoting medication adherence, reducing adverse drug events, and supporting cost-effective treatment options.

The Pharmacist ensures compliance with regulatory standards and supports the organization's mission of delivering high-quality, patient-centered care.

Core Responsibilities

  • Conduct comprehensive medication reviews to ensure safe and appropriate medication therapy
  • Evaluate medication regimens to identify potential drug interactions, contraindications, or duplications
  • Collaborate with providers and clinical care teams to optimize medication therapy plans
  • Provide clinical recommendations supporting evidence-based medication use

QUALIFICATIONS AND EDUCATION

  • Doctor of Pharmacy (PharmD) or Bachelor's Degree in Pharmacy required
  • Active Pharmacist license in the Commonwealth of Virginia required
  • Minimum of 2 years of clinical pharmacy experience preferred
  • Experience supporting medication therapy management programs preferred
  • Knowledge of Medicare, Medicaid, and healthcare regulatory requirements preferred
  • Strong knowledge of pharmacology and medication safety practices
  • Experience working with electronic medical records (EMR) systems preferred
