ResponsibilitiesWhat You'll Do:- Evaluate patients and develop personalized care plans focused on functional independence, safety, and quality of life
- Deliver care through therapeutic activities, ADL training, and adaptive techniques
- Help patients improve their ability to perform daily tasks such as dressing, bathing, grooming, and meal preparation
- Quickly integrate with interdisciplinary teams, caregivers, and nursing staff to drive meaningful patient outcomes throughout your assignment
- Track patient progress, maintain accurate clinical documentation, and adapt treatment plans based on patient needs and rehabilitation goals
Qualifications- Licensed Occupational Therapist (OT) or license eligible in the state of practice
- Graduate of an accredited Occupational Therapist Program
- Interest or experience in skilled nursing, long-term care or geriatric rehabilitation
- Passion for patient-centered care and improving quality of life for older adults
- Willingness to travel to multiple TMC locations as needed
