What You'll Do:- Evaluate and treat patients in short-term rehab, long-term care, and outpatient settings
- Develop individualized, evidence-based treatment plans
- Deliver skilled therapeutic interventions that improve function and independence
- Collaborate closely with interdisciplinary care teams
- Educate patients and caregivers throughout the rehab journey
- Track patient progress and adapt treatment plans as needed
- Support safe and successful discharge planning
