Transplant Coordinator for Post Liver - Duke University Hospital

Duke University Health System- Duke University Hospital seeks to hire an experienced Transplant Coordinator who will embrace our mission of Advancing Health Together.

The Liver Transplant Department at Duke University Hospital is active in delivering exemplary care for each patient to ensure an excellent patient experience, every time.

The Liver Transplant Department at Duke Health is active in delivering exemplary care for each patient to ensure excellent patient experience, every time. Transplant coordinator role has direct patient contact in education and assessing potential transplant candidates in the clinic and virtual. .
  • Educate patients and families regarding all aspects of transplantation care, including but not limited to, arranging follow up appointments, labs, and working with the multidisciplinary team on patient specific needs in the post - transplant phase.
  • Communicate with patients and caregivers regarding the plan of care and ongoing changes.
  • Maintain case load of post -transplant patients to ensure timely care.
  • Provide for continuity of care between inpatient care, outpatient care, and long term management.
  • Ability to make clinical judgements based upon synthesis of current research, concepts, principles, theories, and outcomes to solve complex problems.
  • Collaborate with medical faculty, pharmacy, nursing personnel, case management and other health disciplines to facilitate meeting patient and family needs.
  • Staying current on transplant research and applying evidence based research and recommendations to patients/families on care of the transplant patient.
  • Complete regulatory documentation in UNOS data base / hospital EMR.
  • Reviewing education materials and working with department if there are changes in current practice.
  • Management of immunosuppression plan per protocol/ medical team.ongoing education with patients and caregivers.

The transplant coordinator is an integral member of a larger interdisciplinary team contributing to excellence in patient care, research, and teaching and in providing leadership in the organization.
