Shift: Shift 1/9:00:00 AM to 7:30:00 PM

Minimum to Midpoint Pay Rate: $51.54 - $67.01 / hour

As a Transitions of Care Pharmacist, you'll serve as the key liaison between inpatient and outpatient pharmacy, ensuring patients have a safe, successful transition to post-acute care.

What you'll do:
  • Work confidently across inpatient and outpatient electronic health records
  • Review discharge medication orders and pend the discharge med rec
  • Educate patients on medications and disease states prior to discharge
  • Ensure patients have access to medications post-discharge
  • Coordinate with outpatient pharmacy on discharge prescriptions
  • Review follow-up appointments and referrals for completeness
  • Coordinate with Care Management on post-discharge needs
  • Follow up with patients after discharge to assess unmet needs

You'll also carry out core pharmacist duties - preparing, dispensing, and monitoring medications across the facility, maintaining a safe and effective medication-use environment for patients of all ages, and providing direct supervision of technical support staff. All duties are performed in accordance with applicable state and federal law, regulatory standards, and health system policy.

What you'll bring:
  • Doctor of Pharmacy degree required (BS Pharmacy accepted with equivalent, acceptable experience from an AACP-accredited College of Pharmacy)
  • Completion of a pharmacy internship
  • Active Florida Pharmacist License
