Full Job Description
ResponsibilitiesEvaluates & assesses all assigned patients, participates in provider-to-provider and family consultation and treatment planning that leads to achievement of outcomes specified in the Individualized Family Service Plan (IFSP). Provides on-going developmental therapy and global evaluations.Evaluation & Assessment, IFSP development, consultation and treatment planning for children in the EI system.On-going developmental therapy that enhances skill development in a variety of developmental areas.Special instruction activities defined in the IFSP to promote the child's acquisition of skills in a variety of developmental areas including cognitive processes and social interaction and curriculum planning.Provision of information and support related to enhancing the skill development of the child that enables to child to attain maximum functional level.Provides assistance with acquisition, retention or improvement in skills related to activities of daily living such as feeding and dressing, communicating with caregivers, and the social and adaptive skills to enable the child to reside in their home or non-institutional community setting.Orientation/mobility and other developmental therapy services related to visual functioning including orientation and mobility training for all environments, communication skills training, visual training, independent living skills training and additional training necessary to activate visual motor abilities.Family training to assist the family of an enrolled child in understanding the special needs of the child and enhancing the child's development.Completion and submission of required documentation in a timely manner.Provision of services in accordance with authorizations issued by Child Family Connections.Participation in continuing professional education activities as required to maintain certification in good standing.Performs other related duties as assigned. Qualifications* Part C EI Services System enrolled EI Specialist, credentialed as a Developmental Therapist (see 89 Illinois Administrative Code, Ch IV, Section 500, Appendix C, Subchapter e: "Requirements for Professional and Associate Level Early Intervention Credentialing and Enrollment to Bill").* Minimum of Teacher Endorsement in Early Childhood Education or Special Education or Bachelors Degree in ECE, Early Childhood Special Education, Special Education, or human service field OR a current Illinois license in art, music, recreation, or other therapy, rehabilitative or habilitative in nature.* Part C EI Services System enrolled EI Specialist, credentialed as a Developmental Therapist and Evaluator.* Minimum of one year experience working hands-on with children aged 0-3 who are developmentally disabled.* CPR certification.
