What We're Looking ForTrillium Health Resources is seeking an experienced Support Transition and Reentry (STAR) Manager to join the Support Transition and Reentry (STAR) Team. STAR is a specialty care team serving incarcerated adults with complex behavioral health conditions. This leadership role provides direct supervision and oversight to ensure the delivery of high-quality, person-centered integrated care. The ideal candidate has experience leading a team, care management expertise, and is familiar with criminal justice systems, reentry programs and community-based services.
On a typical day, you might:- Provide direct supervision and support to QP level coordinators, licensed clinicians and certified peer support specialists.
- Monitor team's quality of work and take corrective action as needed.
- Oversee complex cases ensuring sound decision making and wholistic client care.
- Support staff to effectively plan and coordinate member transitions from incarceration to the community.
- Collect data, assign cases, and conduct quality audits.
- Collaborate with internal departments and external systems including prisons/jails, probation/parole, BH/SU providers, and other stakeholders.

QualificationsRequired:
- Associate's degree with three (3) years of experience working with behavioral/mental health, substance use and/or intellectual/developmental disability populations; OR
- Bachelor's degree with one (1) year of experience working with behavioral/mental health, substance use and/or intellectual/developmental disability populations; OR
- Equivalent combination of education/experience.
- Must have a valid driver's license.
- Must reside within 75 miles of Trillium's Catchment of North Carolina. Counties included: Anson, Bertie, Beaufort, Bladen, Brunswick, Camden, Chowan, Carteret, Columbus, Craven, Currituck, Dare, Duplin, Edgecombe, Gates, Greene, Guilford, Halifax, Hertford, Hoke, Hyde, Jones, Lee, Lenoir, Martin, Montgomery, Moore, Nash, New Hanover, Northampton, Onslow, Pamlico, Pasquotank, Pender, Perquimans, Pitt, Randolph, Richmond, Robeson, Sampson, Scotland, Tyrrell, Warren, Washington, Wayne, Wilson.
- Must be able to travel within catchment as required.
Preferred:
- Human Services or Criminal Justice degree.
- Master's degree.
- Minimum of two (2) years' experience supervising a team (2 or more employees).
- Minimum of one (1) year working in criminal justice or juvenile justice systems.
- Provisionally or fully licensed by the state of North Carolina as a LCSW, LPA, LCMHC, LMFT, or LCAS.
