Full Job Description
JOB SUMMARY:

This position works with nursing team to coordinate care for new patients. Monitors the ongoing home health nurse/patient relationship and patient progress. Directly involved in hiring, coaching, and counseling new field staff, provides orientation and educational training. The position will include establishing and maintaining positive relationships with patients, home health agencies and referral sources. Interacts with the intake department, clinical pharmacists, sales team, administrative departments, and outside agencies. Works with branch leadership to manage performance improvement program of the agency such as chart audits, formulation of Plan of Care (POT/485) and assessing staff's clinical documentation technique, compliance with documentation guidelines and adherence to nursing standards.

MAJOR DUTIES AND RESPONSIBILITIES:

1. Communicates effectively with a variety of people, both verbally and in writing.

2. Coordinates with intake department to obtain appropriate clinical information and coordinate nursing care for new infusion patients.

3. Assists with patient scheduling needs.

4. Serves as primary contact/liaison between KabaFusion branch, and various home health agencies.

5. Interacts with home health agencies and/or infusion agencies in providing authorizations for skilled nursing visits.

6. Develops cooperative relationships with nursing staff, home health agencies, referral sources, and KabaFusion accounting department.

7. Maintains knowledge of current state Home Health Regulations and other accrediting body standards; ensures compliance with both.

8. Ensures agency's continuous compliance with the State Home Health Regulations and ACHC Standards in the operation of a home health agency.

9. Assists DON in creating an environment in which home health staff can work together, in which each has an opportunity for self-expression, for creating a sense of accomplishment and for personal growth.

10. Maintains adequate knowledge of documentation requirements in order to serve as resource for staff.

11. In collaboration with DON interviews and employs qualified personnel. Assures completion of staff evaluations. Performs disciplinary action as required.

12. Participates in providing orientation and clinical competency for new nursing staff, and field competency visits as needed.

13. Assist with patient home visits as needed and/or may carry a caseload.

14. Oversees day to day delivery of skilled nursing visits and agency operational functions.

15. Evaluates and intervenes in nursing-related patient care issues.

16. Assists DON in conducting staff meetings, in-services, and other meetings as assigned.

17. Works cooperatively with professional health members, conducts team conference and provides for continuity of care and coordination among disciplines and other departments.

18. Assures accurate, current medical records are maintained according to state and federal requirements, maintains patient confidentiality.

19. Acts as liaison between the physician, nurse, pharmacist and patient/family when problems arise.

20. Assists DON in maintaining personnel files according to policy and laws.

21. Manages communications to staff regarding changes in agency policies and procedures as directed.

22. Works cooperatively with other home health agencies and/or infusion agencies in coordination of care for skilled nursing visits as needed.

23. Participates in quality management programs and performance improvement based on the Agency needs.

24. Provides case conferences and in-service education programs for all field staff.

25. Assesses daily staffing needs in coordination with team, works with team to adjust existing personnel and/or notifies personnel to report to ensure proper staffing within a timely manner. Reviews schedules for appropriate coverage.

26. Participates in coordination of on-call services during non-office hours as needed, performs "oncall" duties as needed or assigned.

27. Other related duties as assigned.

EDUCATION AND/OR EXPERIENCE:
• Graduate from an accredited school of nursing. BSN and/or Public Health Nurse Certificate preferred.
• Current and unrestricted registered nurse licensed in the state of practice.
• Active driver's license and ability to provide current auto insurance
• Current BLS/CPR card
• Minimum of three years' experience as a registered nurse in a home health agency, home infusion pharmacy or its equivalent.
• Minimum of two years supervisory experience in a health care setting preferred.
• Two years of supervisory experience in a health care setting required if mandated by state regulations.
