Full Job Description
The Stars Population Health Strategy Lead develops and executes population health strategies that improve outcomes for defined member populations. This role identifies opportunities through data analysis, designs evidence-based initiatives, monitors performance, and partners with internal and external stakeholders to address gaps in care, reduce disparities, and support quality improvement.
The ideal candidate is a strategic thinker with strong analytical, communication, and program management skills. They should be comfortable translating complex data into actionable insights, influencing cross-functional partners, and presenting clear recommendations to leadership.

Health Quality and Stars is focused on improving health outcomes and enhancing the care experience for members and provider partners through quality-driven solutions. The organization supports high plan quality as measured by the Centers for Medicare & Medicaid Services through the Five-Star Quality Rating System.

The Five-Star Quality Rating System evaluates Medicare Advantage and Prescription Drug Plans across measures such as preventive screenings, chronic condition management, health outcomes, patient experience, and plan operations.

The Stars Population Health Strategy Lead will be responsible for developing population health strategies, implementing targeted initiatives, monitoring performance, and partnering with internal and external stakeholders to improve outcomes for priority populations.

Key Responsibilities

  • Design and implement population health initiatives that address health risks, disparities, and gaps in care.
  • Analyze population data, quality measures, and performance trends to identify improvement opportunities.
  • Develop strategies that support improved health outcomes, member experience, provider engagement, and quality performance.
  • Monitor initiative performance and evaluate program effectiveness against targeted outcomes.
  • Adjust strategies based on data insights, operational feedback, and evolving business priorities.
  • Build and strengthen relationships with key stakeholders in member-facing, provider-facing, clinical, operational, and analytics roles.
  • Partner across teams to align priorities, remove barriers, and support successful execution of population health initiatives.
  • Develop holistic operational reporting, including performance insights, risks, opportunities, and executive-level summaries for Stars leadership.

Required Qualifications

  • 8 or more years of experience in strategy development, implementation, operations, consulting, clinical programs, or a related business area.
  • Bachelor's degree or equivalent work experience
  • Experience analyzing complex data and using insights to develop strategies, recommendations, and compelling narratives.
  • Ability to interpret and apply data and analytics to improve performance and guide decision-making.
  • Experience in strategy consulting, business strategy, clinical operations, health care operations, or related areas.
  • Strong presentation skills, including the ability to communicate clearly with leadership and cross-functional stakeholders.
  • Demonstrated ability to communicate effectively in written and verbal formats.
  • Proven ability to facilitate discussions, influence stakeholders, and drive alignment across teams.

Preferred Qualifications

  • Masters degree in Public Health, Business Administration, Health Administration, or a related field.
  • Experience in a health care, managed care, Medicare Advantage, quality improvement, or Stars-related environment.
  • Agile, product management, or program management certification.
  • Strong analytics background, including experience with performance reporting, population health metrics, or quality measures.

Pay Range

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.


$115,200 - $158,400 per year


This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
