Description:
Provide assessment, diagnosis, and treatment of speech, language, cognitive-communication, and swallowing disorders in a geriatric, medically complex population in a subacute/long-term setting.
Collaborate with the interdisciplinary team to support quality patient outcomes and ensure regulatory compliance.
Conduct comprehensive evaluations within 24 hours of physician referral to assess speech, language, cognition, and swallowing.
Develop a functional therapy diagnosis and prognosis.
Design and implement individualized treatment plans using evidence-based interventions.
Adjust treatment based on progress and establish clear, measurable goals.
Provide direct therapy addressing communication and swallowing disorders, including compensatory strategies and cognitive-communication interventions.
Document accurately and timely, including evaluation reports, daily treatment notes, and discharge summaries.
Educate residents, families, and staff on communication strategies and safe swallowing techniques.
Coordinate care with the interdisciplinary team to optimize patient outcomes and support discharge planning.
Participate in quality improvement and advocacy for patient-centered care and compliance with documentation standards.
If supervising others, ensure compliance with regulations regarding supervision.
Experience in skilled nursing facilities or geriatric/rehabilitation settings preferred.
Strong skills in clinical assessment and treatment planning are essential.
Knowledge of billing and regulatory requirements is preferred.
Flexibility and adaptability are key in managing workloads and therapy sessions.
