This position would cover Skagit County (Mount Vernon, Burlington, Sedro-Wolley, Bow, Anacortes) and part of Whidbey Island (Coupeville, Oak Harbor) for home health speech therapy services.

Duties and Responsibilities- Assumes responsibility for the on-going interdisciplinary assessment and development of the individualized plan of care in partnership with the patient, their family / representative(s) and caregiver(s).
- Provides patient, family and/or caregiver education, counseling, and refers patient to other home health services as appropriate.
- Identifies functional speech defects and establishes a plan of care to improve function.
- Administrates speech therapy program utilizing specialized therapeutic techniques and/or equipment.
- Communicates with the physician responsible for the home health plan of care and other health care practitioners on an on-going basis to inform of patient's condition and progress.
Knowledge, Skills and Experience- Must have a master's or doctoral degree in speech-language pathology and be licensed in the state of practice.
- Must possess and maintain licensure to practice in the state of Washington.
- A minimum of one year's clinical experience is required; home health experience is preferrable.
- Possess and maintains throughout employment an active driver's license, auto insurance policy and basic lifesaving CPR.
Preferred Qualifications- 2+ years of home health experience.
- Familiarity with the Homecare Homebase / Pointcare EMR system.
Schedule, Pay and Benefits- Full-time, 32 hours + a week
- Hourly pay $50 - $60 per hour, eligible for overtime.
- Reimbursed for mileage.
