About the JobPosition Overview:We are seeking a dedicated and compassionate Speech-Language Pathologist (SLP) to provide high-quality therapeutic services to patients in their homes. This position offers the opportunity to make a meaningful difference in patients' lives while enjoying the flexibility and autonomy of home-based care.
Key Responsibilities:- Conduct comprehensive speech, language, cognitive, and swallowing assessments in patients' homes.
- Develop and implement individualized treatment plans based on physician orders and patient needs.
- Provide therapeutic interventions for various conditions including stroke, brain injury, dementia, Parkinson's disease, and other neurological conditions.
- Educate patients and family members on therapeutic techniques and home exercise programs.
- Document patient progress and maintain accurate clinical records in compliance with Medicare and insurance requirements.
- Collaborate with physicians, nurses, and other healthcare team members.
- Ensure compliance with all regulatory standards and company policies.
Required Qualifications:- Master's degree in Speech-Language Pathology from an accredited program.
- Current state licensure as a Speech-Language Pathologist.
- Certificate of Clinical Competence (CCC-SLP) from ASHA preferred.
- Previous experience in home health, skilled nursing, or rehabilitation setting preferred.
- Valid driver's license and reliable transportation.
- Current CPR certification.
