What You'll Do- Evaluate and treat speech, language, cognitive-communication, voice, and swallowing disorders in patients' homes.
- Develop individualized treatment plans based on each patient's goals and daily routines.
- Educate patients and caregivers to promote carryover and long-term success.
- Collaborate with physicians, nurses, physical therapists, occupational therapists, social workers, and the interdisciplinary care team.
- Monitor progress, modify treatment plans, and document clinical outcomes.
What We're Looking For- Active Minnesota Speech-Language Pathologist (SLP) license.
- Strong clinical judgment and evidence-based practice skills.
- Passion for helping patients improve communication, cognition, and swallowing function.
- Home health experience is welcomed but not required.
- Valid driver's license and reliable transportation.
