Full Job Description
Speech Language Pathologist, an established Rehab company with over 35 years of successful business is hiring a Speech Language Pathologist. We are looking for a Speech Language Pathologistto float between Cokato, and Dassel area! We offer a competitive wage, benefits, mileage reimbursement ($.70 / mile) and up to a $2,500 bonus paid out over 18 months. We are seeking an SLP with current CCC-SLP certification.

Position Type: Full-Time or Part-Time, benefits eligible position

Wage Range: $36 - $46 / hour depending on experience.

Bonus: $2,500

Location: West Region: Cokato and Dassel

Speech Language Pathologist Responsibilities:
  • Assess & Diagnose Communication Disorders: Evaluate patient needs and deficits such as but not limited to: patients with speech, language, voice, fluency, and swallowing difficulties.
  • Develop & Implement Treatment Plans: Create personalized therapy programs to improve the functional deficits of the patient.
  • Provide Speech & Language Therapy: Conduct exercises and interventions to improve communication and speech abilities.
  • Educate & Support Patients & Families: Teach strategies for effective communication and techniques to improve daily life.
  • Monitor Progress & Adjust Treatment: Track patient improvements, document progress, and modify therapy plans as needed.
  • Collaborate with Healthcare Teams: Work with doctors, nurses, and therapists to ensure comprehensive support for patients.
Speech Language Pathologist Qualifications:
  • Must be a graduate from an Accredited Speech Therapy Program and have a bachelor's degree.
  • Must possess a current accredited MN Speech Therapist License.
  • Homecare Homebase EMR experience helpful but will train
  • Experience with OASIS preferred or willing to be trained
  • Must possess strong knowledge regarding state, federal, and local regulations as they pertain to Home Health care.
  • Develop care plans and discharge planning strategies for optimal patient recovery.
  • Collaborate with healthcare professionals to ensure comprehensive and personalized treatment plans.
  • Demonstrate excellent communication skills to educate patients and their families on therapy techniques and home care practices.
  • Must be capable of performing the essential functions of the job, with or without reasonable accommodations.
  • Must be able to communicate in English, both verbally and in writing and possess sufficient communication skills to perform the tasks required.
