The compensation offered to the candidate selected for the position will depend on several factors, including the candidate's educational background, skills and professional experience.
As a Social Worker MSW, a typical day might include the following:
- Identifying, screening, and completing comprehensive clinical assessments for patients across assigned hospital populations.
- Formulating and implementing timely, appropriate, and safe discharge plans tailored to each patient's unique needs.
- Providing supportive counseling to patients and their families regarding illness, coping with hospitalization, and care transitions.
- Connecting patients with critical community resources, medical equipment, or post-acute care, and conducting diligent follow-ups.
- Participating actively in interdisciplinary team rounds to review caseloads, align care paths, and advocate for patient needs.
- Coordinating social work services within the palliative care team and facilitating patient-family meetings for goal setting and advanced care planning.
- Serving as a liaison between hospital clinical teams, quality improvement, community hospices, home care agencies, and long-term care facilities.
- Entering accurate, clear, and timely clinical documentation and intervention records into the patient's electronic medical chart.

To be considered for this opportunity, you must have:
- A Master's degree in Social Work (MSW) from an accredited School of Social Work.
- An active Social Work Licensure in the State of New Jersey (LSW or LCSW).
- The flexibility to participate in a weekend and holiday work rotation alongside your team.
- Strongly prefer previous medical social work or hospital case management experience.
- Bilingual language skills in English and Spanish are highly preferred.
- Eligibility or interest in providing field instruction for graduate or undergraduate social work students.
