Responsibilities

• Provides a full range of Social Work Services to patients from admission to discharge.  
• Participates in the interdisciplinary team care process. 
• Interacts effectively with external agencies in regard to discharge planning issues.   
• Advocates for the optimal discharge plan for 3-K patients. 
• Leads therapeutic groups for patients.   
• Provides clear and comprehensive documentation.. 
• Communicates effectively and with the interdisciplinary team.  
• Is knowledgeable about the intricacies of local discharge planning options. 
• Maintains productive relationships with patients.
• Is self-directed and able to work independently as needed. .

Requirements:

• Masters Degree in Social Work from an accredited college/university
• Licensed in NYS as an LMSW
• Two years experience in a social service agency preferred. 
• Psychiatric social work experience preferred. 
• Knowledge of community resources.  
• Demonstrated leadership skills.
