Full Job Description
SUMMARY

The Social Worker I provides direct psychosocial support, discharge planning, and care coordination for patients and families in collaboration with physicians, case managers, and the interdisciplinary care team. This role assesses patient needs, identifies barriers to care, develops treatment and transition plans, and connects patients with appropriate community and post-acute resources to promote optimal outcomes.

ESSENTIAL FUNCTIONS
  • Conduct psychosocial screenings and full assessments for patients requiring social work intervention.
  • Develop and implement individualized care plans focused on psychosocial support, discharge planning, and transition of care.
  • Collaborate with the interdisciplinary team to support care coordination and patient progress.

  • Maintain timely, accurate documentation in accordance with departmental and regulatory standards.
  • Participate in interdisciplinary rounds, quality improvement activities, committees, and on-call/coverage responsibilities as assigned.
  • Perform other duties as assigned.


QUALIFICATIONS

Education
  • Master of Social Work (MSW) required

Licensure

Salary Range: Minimum 32.40/hour - Maximum 53.48/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
