Full Job Description
SUMMARY

In collaboration with patients, families, case managers, physicians, and the interdisciplinary team, the Social Worker I provides psychosocial assessment, counseling, care planning, and discharge/transition support. Responsibilities include identifying and addressing psychosocial barriers to care, coordinating post-acute services, educating patients and families, and implementing interventions to support optimal outcomes.

Specific Duties and Responsibilities Provides comprehensive psychosocial assessments, diagnostic formulations, and treatment (individual, family, and group psychotherapy) for patients in the Adult Schizophrenia Program and Clozapine Clinic within the Community Psychiatry Program at Johns Hopkins Bayview. Therapist will also provide crisis intervention when appropriate, maintain clinical documentation in electronic record, feedback informed care and obtains insurance authorizations for services when necessary. Work is performed on a multi-disciplinary treatment team consisting of therapists/social workers, case manager, psychiatrists, and PRP staff. Additional duties may be some research-related items as well as learning and providing feedback informed treatment with clients.

Education: Master Degree in Social Work, Psychology or other related disciplines

Knowledge, Skills, and Abilities:

Strong interpersonal and collaboration skills.

Knowledge of community resources.

Ability to assess, synthesize, and summarize clinical and psychosocial information.

Strong oral and written communication skills.

Ability to prioritize work effectively and efficiently.

Proficiency with electronic health records, email, and office/communications systems.

Professional Licensures/Certifications LGPC/LCPC, LMSW/LCSW-C, LGMFT/LMFT

Experience
  • Up to one year of healthcare-related experience preferred.
  • Field placement may be considered in lieu of experience.

Salary Range: Minimum 32.40/hour - Maximum 53.48/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
