Social Worker / Behavioral Health Clinician - Primary Care

Salary Range: $69,535-$102,877/hr based on experience and qualifications

About the Role

We're looking for a dedicated Social Worker / Behavioral Health Clinician to join our team and help us continue delivering the level of care our patients and families deserve. In this role, the Behavioral Health Clinician participates as an integral member of a multi-disciplinary care team in one or more primary care settings. This position provides clinical services including screenings and comprehensive assessments to individuals with mental health or substance use disorder problems. The Behavioral Health Clinician provides evidence based psychotherapy services to patients and at times, their families. This position will also provide treatment planning, discharge planning, and referral to services.

What You'll Do

Psychotherapy: Provides screening and comprehensive assessments to primary care patients within the practice. Based on the clinical formulation and diagnosis will provide psychotherapy services with an emphasis on evidence based short term therapy models. Will provide longer term therapy as needed or refer to community based providers. In collaboration with the patient, develops treatment plans and discharge plans. Provide crisis intervention services as needed. Demonstrates excellence in both internal and external customer service.

Referral Management: Initiates or accepts referrals from staff for behavioral health assessments and makes a level of care determination. Will assist with referrals for community based behavioral health services and other services as indicated.

Case Management: Provides time limited case management services to patients on active caseload. Coordinates case management with nursing staff as appropriate. Provides consultative services to providers and other members of the staff.

Documentation: Exhibits knowledge and skill in documentation of all services in the electronic health record. Ensures that comprehensive assessments, treatment plans, discharge plans, progress notes and documentation of case management activities are completed in accordance to standard and in a timely way.

Outreach Activities: Staffs and/or coordinates staffing of community events, health fairs, etc. in which the primary care practice participates. May participate in public relations or informational presentations to community agencies, businesses, services and other organizations. Creates and maintains promotional materials and adequate inventory.

Community Network and Resources: Established and maintains positive working relationships within the community agency service system. Establishes and maintains effective network of community resources for cross-referral of patients. Demonstrates ability to continuously develop both external and internal resources to support needed referrals and coordination of care.

Integration: Promotes integration of behavioral health services into the primary care model through identification of patient-specific interventions and provisions of evidence based services and serves as "internal consultant" and "champion" to providers and staff through evaluation of potential programs and their impact on behavioral health.

What You Bring
  • LCSW OR LMHC required. Minimum of two years' clinical practice in an ambulatory setting with at-risk patients preferred.
  • Knowledge of available community resources and the ability to evaluate resources and make appropriate referrals.
  • Knowledge of health-related financial assistance, including Medicare and Medicaid.
