Job Title: Social Worker - Ambulatory Clinic

As a member of the interdisciplinary care team in this multi-specialty care setting, the social worker will support the following medical specialty practices: Clinical Immunology, Endocrinology, Gastroenterology, Infectious Diseases, Renal, and Sleep Medicine

Responsibilities

In this role, the social worker focuses on same-day, brief interventions to address issues such as behavioral health assessment/referrals (including suicidal ideation), and social determinant of health needs such as, homecare, insurance, housing, and hunger. The social worker conducts brief biopsychosocial assessments, identifies mutually agreed upon goals, and assists with developing and facilitating an action plan to address those goals. The social worker is responsible for educating patients and care partners about navigating the health system and the availability of relevant resources, and linking patients/care partners to community organizations and services.

This position will be an on-site position, with the social worker available to the practices Monday - Friday from 9am - 5pm. Referrals to the social worker will come from the multi-disciplinary team of the related specialty. Other means of patient identification may also be used to help ensure patients in need of social work support are connected to care.

The social worker in this position will provide a range of social work services to patients. Responsibilities will include:

• Conducting clinical social work assessments and Social Determinant of Health screening
• Coordinating the psychosocial components of patient care, including but not limited to: assistance with concrete services related tobenefits and entitlements, connection to home care, medication assistance, DME orders/access, transportation, and other SDH needs impacting patient's health outcomes and quality of life
• Providing counseling to patients within the context of unmet psychosocial needs and chronic illness
• Coordination/collaboration with the patient's specialty care team, Care Management, community supports and others involved in patient's care

The patient population in this practice will be diverse both in regards to age/demographics and level of need for social work assistance. Insurance accepted by the practice will encompasses both private insurances and Medicare/Medicaid.

Qualifications

• NYS LMSW or LCSW.
• Experience in providing rapid psychosocial assessment and focused, short-term interventions.
• Knowledge of entitlements, resources and the ability to assist patients with concrete needs and provide linkages, referrals, and enhanced patient follow-through by reducing barriers to care.
• Ability to assess needs and coordinate resources with hospital and community networks for effective management of patient care.
• Ability to work both autonomously and as part of a multi-disciplinary team.
• Excellent clinical, collaborative, organizational, and leadership skills.
• Capacity to work with a diverse patient population.
• Bilingual (English/Spanish) preferred.
• Minimum one year in current position.

Compensation Statement

The Mount Sinai Health System (MSHS) provides salary ranges that comply with the New York City Law on Salary Transparency in Job Advertisements. The salary range for the role is $88,691.00 - $101,670.78 Annually. Actual salaries depend on a variety of factors, including experience, education, and operational need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
