Job Summary: The Site Medical Director (SMD) provides clinical leadership, clinical oversight, and administrative direction for providers delivering care at the VGMHC 7th Avenue Acute Care Clinic. The SMD is responsible for supervising physicians and advanced practice clinicians (APCs), promoting evidence-based practice, supporting provider development, standardizing clinical workflows, and advancing quality improvement initiatives across assigned service lines. The SMD partners closely with the Chief Medical Officer (CMO), Senior Medical Director of Provider Affairs (SMDoPA), operations leaders, and interdisciplinary teams to ensure high-quality, equitable, trauma-informed care across programs.
Essential Duties and Responsibilities:
• Participate as a member of the Acute Care Clinic's leadership team to:
o Solve clinic problems (schedules, coverage, etc.).
o Implement improvement and other projects at the clinic level.
o Perform responsibilities delegated by senior leadership.
o Implement plans to meet operational, financial, and patient care quality goals, including monitoring of progress toward relevant quality metrics.
o Attend the local and administrative meetings as needed and determined by the Chief Medical Officer (CMO) or Senior Medical Director of Provider Affairs (SMDoPA), or site leadership.
o Provide clinical input to assist in solving patient issues related to quality of care, behavioral issues, etc.
o Directly supervise physicians and advanced practice providers assigned to the 7th Avenue clinic.
o Resolve personnel issues.
• Provide clinical leadership and mentoring of clinicians at their site in the practice of evidence-based, timely, effective, efficient, and equitable medical care and:
o Provide clinical input on care and management of individual patients as may be needed by colleagues and co-workers.
o Review cases for CSOC (Controlled Substances Oversight Committee).
o Review unexpected outcomes, including deaths, as part of incident peer review and quality assurance and improvement.
o Participate in ongoing professional practice review of direct reports
• Attend Administrative/Quality Improvement Meetings.
• Organize the site's provider meetings, including tracking of minutes, and assist with organizing Staff and Team meetings, as well as ensuring appropriate follow-up.
• Assist in coordination of provider scheduling, including approval of vacation and other time off requests, and ensuring timecards are approved and time management of providers is monitored.
• Act as the clinical liaison between the 7th Avenue Acute Care site and other Virginia Garcia departments, including Dental, Pharmacy, Nursing, Clinical Training Team, Behavioral Health, Population Health, School-Based Health Centers, and community partners, to ensure coordinated, integrated patient care.
• Attend to the day-to-day concerns of providers, support staff and management team that may arise.
• Ensure that procedures are in place to maximize the flow of information through the clinic to increase patient safety and provider continuity.
• Assist in implementation of plans to meet the objectives of the annual "Health Plan" of the federal grant and other goals established by the QA/QI committee and the health plans and funders.
• Ensure identified training needs are addressed for both providers and support staff.
• Lead provider annual performance reviews and provides ongoing feedback to direct reports.
• Have periodic 1:1 meeting with direct reports at least quarterly.
• Work with the SMDoPA and local leadership teams on recruitment of providers and other medical staff for site as appropriate, scheduling time for interviews with providers, lunches, etc.
• Orient new providers to the site and conduct regular check-ins to support their onboarding and integration.
• Act as a representative of site's medical staff with site and senior leadership, assuring good communication and acting as a liaison across sites.
• May be required to temporarily assume responsibilities of the CMO, another site medical director or the SDoPA in their absence.
• Maintain clinical competency and provide direct patient care, or provider support as operationally necessary to support onboarding, quality initiatives, workflow development, or continuity of patient care.
• Monitor provider productivity, access, and clinical quality metrics while partnering with operational leadership to improve patient access and care delivery across assigned service lines.
• Provide timely (as defined by VG documentation standards) documentation in the medical record of care provided to patient, including education, medications, referrals, diagnostic testing and follow-up.
• Provide supervision and education of students and residents in the health care professions.
• Assist in organizing special clinics, such as the farmworker outreach program.
• While this position will be assigned to a primary location, the SMD will be required to travel and provide medical care and supervision at various VG and community locations.
• Perform other duties as assigned.
• Handle protected health information (PHI) in a manner consistent with the Health Insurance Portability and Accountability Act of 1996 of (HIPAA).
HIPAA Requirements:
The Site Medical Director will have access to PHI in the course of carrying out their duties. The Site Medical Director uses the PHI for treatment purposes and quality assurance operations. Applying the minimum necessary standard of HIPAA, the designated record set to which this employee will have access include demographic and scheduling information in the practice management system, and the full medical record.
Knowledge, Skills and Abilities Required:
• Active board certification in Family Medicine or Internal Medicine.
• Licensed Oregon physician with current DEA registration and a dispensing license.
• At least 4 years and preferably 6 years of prior clinical experience required with 1 year provider supervision experience in community health preferred.
• As a condition of employment for a medical or dental provider at VGMHC, these employees agree to provide services to non-health center patients under the following circumstances:
• Community-wide special clinics at VGMHC School-based clinics and other school linked clinics
• Health Fairs
• Immunization Campaigns
• Farmworker Outreach Programs
• Homeless Outreach
• Hospital-related activities - for providers with hospital privileges, cross coverage for patients of the community in the hospital is expected
• Coverage of certain individual emergencies - community members may experience a medical emergency in or around one of our health centers. VGMHC medical providers are expected to respond medically appropriately to these emergencies.
Education and Experience Required:
• Active board certification in Family Medicine or Internal Medicine.
