Job Summary:
The Sepsis RN Coordinator/Patient Navigator RN EDSP is responsible for the strategic development, implementation, and ongoing management of protocols, procedures, and performance improvement initiatives related to sepsis. This role ensures early identification, appropriate treatment, accurate documentation, and improved outcomes for patients with actual or suspected sepsis across the Emergency Department, inpatient units, and ICU.
In addition, the Sepsis RN Coordinator/Patient Navigator RN EDSP supports the Emergency Department Screening Program (EDSP) by facilitating routine opt-out screening follow-up for HIV and Syphilis. This role focuses on patient education, results disclosure per protocol, care coordination, linkage to treatment, and collaboration with internal and external partners to ensure continuity of care after ED visit.

Serving as a clinical leader, educator, liaison, and data analyst, the coordinator plays a key role in aligning care practices with Joint Commission certification standards, California Title 22, CMS Core Measures, and hospital goals and serves as key liaison between Emergency Department staff, patients, community clinics, and public health partners to reduce barriers to care, and improve health outcomes.

Essential Functions:
Leads the development, implementation, and continuous improvement of evidence-based sepsis care, along with workflows, protocols, policies, and procedures across Emergency Department and inpatient departments.
Coordinates interdepartmental communication and planning to support compliance with Sepsis Metrics (SEP-1, Mortality, Sepsis LOS, Sepsis readmission rates and bundle compliance)
Provides education and training to staff-including nurses, physicians, and support personnel-on sepsis recognition, early intervention, treatment protocols, and documentation standards.
Implements and monitor screening tools and procedures to identify patients at risk for sepsis, facilitating early detection and timely care.
Collects, analyzes, and reports sepsis-related data to identify trends, monitor outcomes, and support performance improvement initiatives.
Collaborates with interdisciplinary teams to integrate sepsis protocols into patient care pathways and ensure seamless coordination across departments.
Maintains current knowledge of Joint Commission, CDPH, CMS and California Title 22 standards related to sepsis care.
Oversees compliance tracking and reporting for sepsis protocols, including submission of data to regulatory bodies and internal leadership.
Participates in quality improvement initiatives aimed at reducing sepsis-related morbidity and mortality.
Promotes community education and awareness about sepsis through outreach programs, workshops, and seminars.
Advocates for and normalize routine opt-out screening and treatment for HIV and syphilis within the Emergency Department, including supporting the use of visible signage and patient outreach materials.
Establish and maintain referral pathways with community partners, including Ryan White clinics, Federally Qualified Health Centers (FQHCs), public health departments, and community-based organizations.
Serves as a primary point of contact for EDSP follow-up by responding to calls, emails, messages, and texts from ED staff, community providers, local health departments, and patients.
Provides patient education and result disclosure related to STIs and HIV in alignment with provider-directed treatment plans and hospital policy.
Facilitates timely linkage to care, including coordination of confirmatory testing, treatment initiation, and follow-up appointments.
Maintains ongoing communication with patients to support appointment adherence, address barriers, and encourage engagement in care.
Implements and maintain systems to confirm treatment completion and successful linkage to ongoing care, in collaboration with public health and community partners.
Connects patients to supportive services addressing social determinants of health (e.g., transportation, housing, food insecurity, mental health, substance use treatment).
Completes timely, accurate documentation in the electronic health record and required data collection systems.
Aggregates and report EDSP metrics (monthly/quarterly), as required, to support program monitoring, reporting, and quality improvement.
Uses clear, concise, and professional communication with coworkers, patients, and internal/external customers.
Applies AIDET principles in patient and family interactions and utilizes whiteboards for effective communication.
Collaborates across disciplines to assist in coordination of patient care, including transfers, discharges, referrals, and addressing psychosocial/spiritual needs.

Demonstrate knowledge of evidence-based public health practices and current clinical guidelines related to Syphilis and HIV consistent with EDSP principles.
Educate staff on best practices for documenting sexual orientation and gender identity (SOGI) and addressing patients in alignment with their identities.

Qualification/Experience:
Three (3) years' experience as a nurse in an acute care setting.

Licensure/Certifications:
