Experience Required: 5+ years in physical therapy, 2+ years in home health
Essential Qualifications: - Current Colorado PT license in good standing
- 5+ years clinical experience (home health strongly preferred)
- 2+ years in a mentorship or supervisory capacity
- Expertise in geriatric rehab, post-surgical recovery, and complex mobility cases
- Proven outcomes with Medicare quality measures
Your Advanced Skillset: • Advanced manual therapy techniques
• Comprehensive fall risk assessment and prevention
• Wound care collaboration (with nursing team)
• Family/caregiver education strategies
• OASIS documentation expertise
Your Impact Will Include: Clinical Excellence (70%): - Manage a specialized caseload of 18-22 patients weekly
- Provide cutting-edge interventions for:
- Post-hospitalization recovery
- Neurological rehabilitation
- Joint replacement protocols
- Chronic condition management
Leadership (30%): - Mentor 3-5 staff PTs through:
- Monthly case reviews
- Joint visits (2-3x/month)
- Best practice development
- Participate in quarterly quality improvement initiatives
- Serve as clinical liaison with physician partners
Compensation & Benefits: - $105,000-$135,000/year (based on experience and caseload complexity)
