We are seeking a compassionate and experienced RN Care Manager to support members with complex medical and long-term care needs. In this field-based role, you will coordinate care, develop personalized service plans, and work closely with members, caregivers, providers, and community resources to improve health outcomes while ensuring high-quality, person-centered care.
What You'll Do
- Assess the needs of high-risk and medically complex members to develop individualized care plans
- Coordinate care across providers, specialists, caregivers, and community organizations
- Monitor member progress, identify changes in condition, and update care plans as needed
- Advocate for members by ensuring access to appropriate healthcare services and benefits
- Educate members and caregivers on disease management, treatment plans, available resources, and healthcare benefits
- Review referrals, assessments, and clinical documentation to support care coordination
- Identify trends and opportunities to improve quality, outcomes, and regulatory compliance
- Coordinate service authorizations for long-term care services and community resources
- Conduct home and community visits to assess member needs and provide ongoing support
- Maintain accurate documentation in compliance with state, federal, and organizational guidelines
- Collaborate with interdisciplinary teams to improve care delivery and member outcomes
- Mentor and support new clinical staff during onboarding and training as needed
Required Qualifications
- Active Registered Nurse (RN) license in the applicable state or Compact State License, or active Nurse Practitioner (NP) license
- Graduate of an accredited School of Nursing or Bachelor's degree with 4-6 years of related healthcare experience
- Experience in care management, case management, service coordination, or long-term care
- Knowledge of complex medical conditions, care planning, and healthcare coordination
- Strong communication, organization, and problem-solving skills
- Ability to work independently in a field-based environment while managing multiple priorities
- Proficiency with clinical documentation and electronic health record systems
Preferred Qualifications
- Bachelor's degree in Nursing (BSN)
- Experience within Managed Care, Medicaid, Medicare, or Long-Term Services and Supports (LTSS)
- Experience working with high-acuity or medically complex populations
- Resource Utilization Group (RUG) certification (or ability to obtain within required timeframe if applicable)
- Experience mentoring or precepting clinical staff
