Senior Clinical Director - Home Health Hospice

We are now accepting applications for a Senior Clinical Director / Nursing Home Administrator (NHA) to lead our team to operational excellence and provide the local community with a place to call home!

Summary/Objective:

Will oversee and manage all clinical functions to ensure the delivery of high-quality, cost-effective home health care services. This role will be responsible for strategic planning, regulatory compliance, operational efficiency, and the development of clinical staff. The Senior Director will work closely with other senior leaders to drive performance, achieve organizational goals, and enhance patient outcomes.

Leadership and Management:
  • Provide leadership and direction to the clinical operations team, including nurses, therapists, and other clinical staff
  • Develop and implement clinical policies, procedures, and protocols to ensure high standards of care and compliance with state and federal regulations
  • Oversee the recruitment, training, and retention of clinical staff, ensuring that they meet the agency's standards for quality and patient care
  • Mentor and support clinical managers and staff, fostering a culture of continuous improvement and professional development


Clinical Operations:
  • Ensure that all home health services are delivered in a timely, efficient, and patient-centered manner
  • Monitor and evaluate clinical operations through joint visits with staff to identify areas for improvement and implement necessary changes
  • Oversee case management and care coordination to ensure that patient care plans are executed effectively and within regulatory standards of care
  • Regularly re-evaluates patient nursing needs through interdisciplinary team discussions


Quality and Compliance:
  • Ensure compliance with all applicable local, state and federal regulations
  • Develop and maintain a quality assurance program to monitor and improve clinical outcomes, including infection control
  • Conduct regular audits and reviews of clinical documentation to ensure accuracy, completeness, and compliance
  • Implement corrective action plans as necessary to address deficiencies or areas of concern


Strategic Planning and Performance:
  • Develop and execute strategic initiatives to improve clinical operations and expand service offerings
  • Analyze clinical and financial data to drive decision-making and improve operational efficiency
  • Work closely with the Executive Director and other senior leaders to set goals, develop budgets, and ensure that clinical operations align with the agency's overall mission and objectives
  • Identify opportunities for growth, partnerships, and market expansion


Patient and Community Relations:
  • Ensure that the agency maintains a high level of patient satisfaction and trust
  • Maintains confidentiality of patient information
  • Follows and adheres to agency policies and procedures implementing HIPAA requirements for the privacy and security of protected health information
  • Uses and/or discloses only minimum amount of Protected Health Information necessary to complete assigned tasks
  • Reports all suspected violation of company's HIPAA policies or procedures to the Executive Director
  • Serve as a liaison between the agency and patients, families, and other stakeholders
  • Address and resolve patient or family concerns regarding clinical services


Additional Duties
  1. Participates in on-call duties as defined by the on-call policy
  2. Ensures that arrangements for equipment and other necessary items and services are available
  3. Instructs, supervises and evaluates home health aide care provided every two (2) weeks


Education and Experience
  1. Graduate of an accredited school of nursing with Bachelor Degree in Nursing required; Master Degree in Nursing, Healthcare Administration, or related field preferred
  2. Current licensure in state and CPR certification
  3. Minimum of 3 years of experience in a clinical operations role, with at least 1 years in a leadership position
  4. Management experience required
  5. Excellent observation, verbal and written communication skills, problem solving skills, and ability to analyze data and metrics to drive operational excellence


Skills, Knowledge and Abilities:

Able to relate positively, effectively, and appropriately with patients, families, community members, and providers. Able to read, write, speak and understand English. Meets all health requirements, as required by law. Demonstrates basic computer knowledge and ability with an aptitude to learn company applications.
