Responsibilities

  • Prepare and maintain the MDS schedule and ensure timely data transmission
  • Support facility quality measures and compliance initiatives
  • Collaborate with leadership to promote quality resident outcomes
  • Serve as the facility's Infection Preventionist and assist with related tasks
  • Provide wound care services as applicable

RNAC Director

Neffsville Nursing and Rehabilitation | Lancaster, PA

Position Type: Full-Time
Pay: Based on Experience

Neffsville Nursing and Rehabilitation is seeking a dedicated and detail-oriented RNAC Director to oversee the resident assessment process and ensure accurate, timely, and compliant completion of MDS assessments, care planning, and related documentation.

This full-time position is ideal for an experienced MDS professional who is committed to quality resident care, clinical outcomes, and regulatory compliance.
Responsibilities
  • Oversee and coordinate the facility's MDS assessment process
  • Conduct resident assessments according to established schedules and in response to changes in condition, hospital readmissions, and other significant clinical events
  • Complete MDS assessments, Care Area Assessments (CAAs), care plans, and supporting documentation in compliance with federal and state regulations
  • Develop, update, and monitor individualized resident care plans in collaboration with the interdisciplinary team
  • Coordinate with Nursing, Therapy, Social Services, Dietary, Activities, and other departments to ensure timely completion of assessments and care planning
  • Accurately observe, assess, and communicate changes in resident condition
  • Prepare and maintain the MDS schedule and ensure timely transmission of MDS data
  • Monitor documentation for accuracy and regulatory compliance
  • Support facility quality measures, audits, surveys, and reimbursement initiatives
  • Collaborate with facility leadership and the interdisciplinary team to promote quality resident outcomes
  • Serve as the facility's Infection Preventionist (IP), assisting with infection surveillance, prevention initiatives, staff education, and regulatory compliance
  • Provide Wound Nurse services, including wound assessments, treatment coordination, documentation, physician communication, and monitoring resident progress, as applicable
Qualifications
  • Current Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in the Commonwealth of Pennsylvania
  • Minimum of one (1) year of prior MDS/RNAC experience in a skilled nursing or long-term care setting
  • Working knowledge of MDS processes, RAI guidelines, CAAs, care planning, and Medicare/Medicaid reimbursement
  • Strong understanding of skilled nursing regulatory requirements
  • Experience with Infection Prevention and/or wound care preferred
  • Strong organizational, communication, leadership, and time management skills
  • Ability to work independently while collaborating effectively with the interdisciplinary team
