Harborview Medical Center (HMC)'s Pediatrics Clinic has an outstanding opportunity as an RN Care Manager.

SCHEDULE
  • 100% FTE, 40 hours per week
  • Day shift


DEPARTMENT DESCRIPTION
Harborview's Pediatrics Clinic provides primary care for infants, children and teens. The clinic utilizes a multidisciplinary staff that is highly collaborative and supportive, working together to achieve the best patient outcomes possible.

Clinical services provided include: general pediatric care (well-child care and sick care)/Care Coordination; hearing and vision screening; immunizations for children from birth through age 21; health education and disease prevention; confidential teen services; injury prevention information; same-day appointments for sick children; developmental evaluations; books for children through the Reach out and Read (ROR) program; safety supplies (bicycle helmets) through Injury Free Coalition for Kids (IFKC) of Seattle; dental screening; and newly arrived refugee medical screening. Ancillary services provided through the clinic include: lactation consultant services; social work services; interpreter services for non-English speaking families; legal counseling through a medical-legal partnership; access to Harborview's Community House Calls program to coordinate care for immigrant and refugee families; and health care financial counseling. In addition, the Harborview Pediatrics Clinic provides specialty care through its Center of Excellence in Foster Care, mental health services (Psychiatry and Psychology), behavioral health integration program (BHIP) and traumatic brain injury (TBI) services. As a hospital-based pediatric clinic, the Harborview Pediatric Clinic has on-site access to laboratory, imaging (CT, ultrasound, X-ray), pharmacy and other general specialty.

RESPONSIBILITIES
  • Collaborate with the interdisciplinary team to identify engage, assess, plan, implement, engage, assess, plan, implement, coordinate, monitor, and evaluate options and services in meeting the needs of newly arrived pediatric patients
  • Work closely with the clinic, community agencies, schools, hospitals, and function across the continuum of care, interfacing with patients, caregivers, providers, and community resources to facilitate the best possible clinical outcomes and optimal level of wellness while using resources appropriately
  • Coordinate and manage the care of newly arrived medically complex children for up to one year (including close partnership with resettlement agencies and Seattle Children's) Role includes coordination of the following: DME, referrals to SCH and other community medical organizations. Collaborate with schools and community based services


REQUIREMENTS
  • BLS
  • Current license to practice as a registered nurse in the State of Washington
  • Two years of pediatric nursing experience in an outpatient/clinic setting
  • Care Coordination/Transition Management (CCTM) or equivalent preferred
  • Must obtain CCTM certification within 12 months of hire


Compensation, Benefits and Position Details

Pay Range Minimum:
$46.97 hourly
Pay Range Maximum:
$87.02 hourly
Other Compensation:
