DESCRIPTION

The Registered Nurse, Certified Diabetes Care and Education Specialist (RN CDCES), provides comprehensive ambulatory nursing care and specialized diabetes education services within the Endocrinology clinic. This role integrates direct patient care, care coordination, and evidence-based diabetes self-management education (DSME), while supporting program growth through accurate documentation and compliant billing practices. The RN CDCES practices at the top of license, partnering with providers across Endocrinology, Diabetes, and Metabolism services to improve patient outcomes, increase access, and enhance clinic efficiency.

Key responsibilities include conducting phone triage, managing patient communication through inbox workflows, and providing longitudinal case management. The RN utilizes nursing-sensitive indicators to evaluate and improve quality of care. Clinical duties include point-of-care testing, specimen collection, medication administration (including injections), and delivery of patient and staff education. The role requires familiarity with diabetes technologies and therapies, including insulin pumps, insulin delivery methods (vials, pens, syringes), glucose meters, and continuous glucose monitors.

The RN CDCES demonstrates a strong commitment to patient-centered care and service excellence in all interactions with patients, families, and team members. Ongoing professional development is expected to remain current with advances in diabetes and endocrinology care.The ideal candidate thrives in a fast-paced ambulatory environment and exercises sound clinical judgment with a high degree of autonomy. Practice is consistent with ANA Standards of Practice and Professional Performance, with independent decision-making conducted within a multidisciplinary care model. Success in this role requires initiative, strong prioritization skills, attention to detail, and the ability to collaborate effectively while maintaining positive, professional relationships with patients, families, and colleagues. Additional duties may be assigned as needed.

MINIMUM QUALIFICATIONS

• Must be a graduate from an accredited school of Nursing.
• RN License issued by the State of California.
• BLS/CPR certification at time of hire with commitment to get BART within six (6) months of hire date.
• Certified Diabetes Care and Education Specialist (CDCES) certification required at hire, or eligibility to obtain CDCES certification within a defined timeframe based on demonstrated qualifying diabetes-care experience.
• Minimum three (3) years of recent, direct clinical experience managing patients with diabetes in an Endocrinology, Diabetes, or other dedicated diabetes specialty setting.
• Minimum three (3) years of hands-on clinical experience supporting patients who use insulin pumps and continuous glucose monitors.
• Demonstrated experience managing or supporting insulin pump platforms, such as Tandem, Omnipod, or Medtronic.
• Demonstrated experience independently reviewing CGM and insulin pump data and making clinical recommendations within provider-approved protocols.
• Proficiency interpreting AGP reports and CGM metrics, including TIR, TBR, TAR, GMI, glycemic variability, and overnight glucose trends.
• Demonstrated experience supporting provider-approved titration of basal insulin, bolus insulin, insulin-to-carbohydrate ratios, correction factors, and non-insulin diabetes therapies.
• Demonstrated experience managing patients with Type 1 diabetes, including patients using pump therapy, MDI, CGM, and automated insulin delivery systems.
• Demonstrated knowledge of hypoglycemia management, hyperglycemia management, diabetic ketoacidosis prevention, ketone assessment, sick-day management, insulin pump failure, infusion-set failure, and backup insulin planning.
• Demonstrated experience providing formal diabetes self-management education, including insulin administration, carbohydrate counting, CGM education, pump education, and patient self-management training.

PREFERRED QUALIFICATIONS

• Bachelor's or Master's degree in Nursing (BSN or MSN).
• Current CDCES certification.
• Minimum two (2) years of recent ambulatory RN experience in a high-volume clinical setting.
• Demonstrated experience with telephone triage, in-basket management, patient communication, and longitudinal case management.
• Prior experience as an RN CDCES in an outpatient Endocrinology or Diabetes specialty practice.
• Experience with diabetes technology prior authorizations, device supply coordination, and related patient access barriers.
• Bilingual English/Spanish.

• Selected candidate may be hired at the CNIII level pending review and approval by the Nurse Executive Council.

Pay Transparency Act

Annual Full Pay Range: $138,330 - $193,892 (will be prorated if the appointment percentage is less than 100%)

Hourly Equivalent: $66.25 - $92.86
