ResponsibilitiesThe Patient Care Coordinator is a Registered Professional Nurse who helps facilitate and direct the delivery of patient care in the Telemetry Department. This position participates in performance improvement/research activities; maintains professional practice standards and clinical expertise; and demonstrates leadership skills. The PCC demonstrates core and unit specific competencies based on the patient population served, and in collaboration with and under the direction of the Nurse Manager , participates in the coordination/implementation of fiscal, human and other resources needed to provide patient care services.
- Demonstrates leadership skills in the coordination and delivery of patient care.
- Develops, coordinates and provides direct care for specific patient assignment as required.
- Communicates effectively with members of the health care team, patients and families in an effort to maximize the quality of patient care and facilitate a positive patient experience.
- Coordinates and collaborates with other team members to facilitate effective throughput on unit.
- In collaboration with the NM and Nursing Director, organizes and directs the delivery of patient care services on assigned unit assuring appropriate levels of care are provided to patients based on acuity.
- Coordinates the unit staff breaks and coverage as needed.
- Supports and participates in the orientation of new personnel to the unit.
- Communicates pertinent patient and administrative information to team members on all shifts.
- In collaboration with the NM and Nursing Director , monitors environment of care issues, assures adequate levels of supplies and that all equipment is available and in proper working order. Notifies appropriate personnel as needed.
- Acts as a clinical expert, role model and resource for staff.
- Assists NM in creation of staffing plans, develops work schedules for unit employees to ensure appropriate staffing levels and staffing mix are maintained.
- Coordinates work as needed and assigns unit staff to specific tasks.
- Participates in the QI process by identifying patient care issues and assists in developing, implementing and monitoring PI measures.
- Performs other related duties as required.
- Performs unrelated duties in times of emergency.
Qualifications- Current NYS license and registration as a Registered Nurse. BSN degree preferred.
- Minimum of three (3) years experience as a staff nurse in an telemetry department.
- BLS, ACLS, PALS required.
- Previous leadership experience preferred.
- Active participation in ongoing professional development.
- Membership on hospital committees preferred.
- Effective communication and time management skills required.
