- Provides direct patient care, evaluates outcomes, consults with other practitioners as required and adjusts nursing care processes as indicated to ensure optimal patient care.
- Monitors medical condition of patients and reports changes to appropriate personnel.
- Reports abnormal diagnostic data to physician/shift coordinator/supervisor as appropriate.
- Demonstrates ability to perform technical nursing procedures/skills of patient care competently, seeking assistance when appropriate.
- Implements patient histories, assesses patients' conditions, and develops individualized care plans for assigned patients. Documents on care plans as patient care evolves, including resolution of problems.
- Assesses patient educational needs and barriers to learning. Implements multidisciplinary patient education care plan and documents patient teaching.
