A Brief Overview The Home Care Intake Coordinator is responsible for obtaining the referrals necessary for the initiation of Home Care services and their successful reimbursement.
What you will do- The ability to comprehend spoken and written English, to communicate with clarity both verbally and in written English, work independently, good organizational skills, good documentation skills, ability to use a telephone and operate a computer.
- Non-combative and consistent behavior, ability to appropriately interact with various patient populations, staff and physicians.
- Evaluates patients referred for home care to determine appropriateness for admission, including but not limited to homebound status, insurance eligibility and skilled need.
- Serves as a liaison to provide patients/families other sources of care/services, including but not limited to private pay services, home care agency, etc.
- Assists in the verification of insurance coverage and provides the information to home care clinical and billing staff.
- Consults with social workers, discharge planners, nurses and other referral sources to establish a coordinated discharge plan.
- Serves as liaison for all referral resources regarding appropriateness of referral, eligibility of services, community resources and home care issues and practices.
- Interprets Home Care policies to patients/families, hospital staff, and referral sources.
- Responds to telephone inquiries regarding information about home care services, makes appropriate calls to obtain referral and/or provides information for alternative community resources.
- Incorporates the clinical history, physiological and psychosocial of patients into the generation of a referral to insure appropriate discipline referral.
- Participates in in-services, continuing education, committees and interdepartmental meetings as assigned.
- Maintains and respects staff and patient confidentiality.
- Demonstrates compliance with the Hospital and Home Care dress code policy.
- Observes hospital and agency policies concerning Fire and Safety regulations, Infection Control Standards, and other rules governing conduct while at work.
- Demonstrates a commitment to work schedule by timely reporting on duty and demonstrating good attendance in compliance with the organization's policies.
- Understands and participates in the Home Care Performance Improvement Plan and process.
- Collects and maintains statistical data on all referred patients and submits as requested to the Director of Home Care.
- Accepts and completes additional responsibilities as directed by management.
Education Qualifications- Associate's Degree RN Diploma Required
Experience Qualifications- 2 years experience in a home care setting Required

In addition to those factors - we believe in the importance of pay equity and consider any internal equity of our current team members as a part of any final offer. Pay Range: $89,440.00 - $110,240.00
