Job Summary and Responsibilities

As our Registered Nurse (RN), you will be a pivotal healthcare professional, delivering compassionate, high-quality care that truly impacts our patients' well-being and recovery.Every day, you will leverage your expertise to provide individualized, comprehensive care, making critical assessments, performing skilled procedures, and meticulously implementing patient care plans.   • Provides effective,comprehensive evidence based nursing care to achieve desired health outcomes. Organizes and prioritizes patient care activities balanced with the needs and comfort of the patient. Collaborates with other disciplines to insure comprehensive, holistic and effective plan of care. Applies knowledge of current practice standards, guidelines, statutes, rules and regulations. Demonstrates accountability for own ethical behavior and supports ethical practice by addressing or escalating issues. Serves as a patient advocate.ASSESSMENT: Completes patient assessments competently. Assesses for and identifies needs of unit-specific patient population: Assesses for and identifies any physical, emotional, psychosocial, spiritual, educational, discharge and cultural needs.PLANNING: Completes an individualized plan of care based on assessment of the patient. Coordinates with other disciplines when indicated. Prioritizes care needs and sets measurable goals and time frames. Includes patient and family in the planning.IMPLEMENTATION: Implements interventions based on nursing and medical plan of care. Manages patient population, diagnosis and age specific needs through appropriate adaptation of interventions. Appropriately utilizes equipment and supplies (including following established policies, procedures and protocols governing the transport, handling and storage of intravenous fluids).EVALUATION: After implementation, reassesses the patient to verify effectiveness of interventions. Updates the plan of care to reflect current care needs.
  • CRITICAL THINKING: Demonstrates the ability to recognize and adapt to changes in patients condition (i.e.. LOC change, vital signs, test results)in a timely and clinically appropriate manner. Uses SBAR to notify physicians and other providers of clinical findings. Follow policies and procedures for contacting physician regarding critical values. Uses appropriate resources and follows chain of command. Recognizes limits of scope of practice. Seeks multidisciplinary collaboration. Integrates & interprets multiple sources of data to make appropriate clinical decisions based on patient needs & desired outcomes.
  • DECISION MAKING:Assesses situations and seeks critical information to make decisions. Implements solutions while reviewing and monitoring effects. Re-evaluates decisions and makes corrective actions when indicated. Makes decisions based on evidence based research. Assumes accountability for decisions made.DELEGATION: Delegates tasks based on the needs & stability of the patient, complexity of the task, outcome predictability and skill level& scope of practice of the staff. Delegates tasks but not responsibility and accountability for completion. Verifies that delegated work is completed. Holds self and others accountable. Uses team skills effectively
  • DIVERSITY & CULTURAL AWARENESS:Uses knowledge of cultural differences to provide patient care that is sensitive to and consistent with cultural values, beliefs and customs. Provides interpretive services for patients and families. Recognizes and utilizes individual differences, diversity and knowledge of the health care team.CLINICAL LEADERSHIP: Recognizes changes in the stability of a patients condition and provides appropriate interventions. Uses information from multiple sources to collaborate, negotiate, coordinate, delegate and evaluate care delivery. Participates in ongoing education. Participates in the development of others through mentoring and precepting activities. Uses negotiation and conflict management strategies. Identifies opportunities for process improvement and actively participates in implementation of changes. Knows and follows hospital and department specific policies and procedures.
  • INFORMATION SYSTEMS/DOCUMENTATION:Documents assessments, plan of care, interventions and reassessments within established timeframes. Documents changes in condition and response to interventions within established timeframes. Documents comprehensive discharge instructions that include diagnosis, home instructions and medications to take. Utilizes electronic documentation system and electronic reporting systems.FAMILY CENTERED CARE/PATIENT SATISFACTION:Connects with and establishes relationships with patients and families using therapeutic communication skills. Takes responsibility and accountability for patient/family satisfaction by using the nursing bundle. Demonstrates AIDET Principles. Demonstrates 8 behaviors of Hourly Rounding and uses the care board when applicable. Assesses patient/family personal needs and provides Individualized Care. Conducts Discharge Phone calls when assigned.Involves patient/family in Shift Bedside Report with oncoming/offgoing nurse. Initiates service recovery when indicated.Uses proper etiquette in person, on the phone & in emails. Assist others by escorting/directing visitors.
  • CONTINUOUS IMPROVEMENT:Identifies, makes suggestions for and/or initiates process improvement with new and innovative ways to improves practice and care delivery. Participates in process and quality improvement activities. Delivers care that meets the goals of the hospital/department/unit. (i.e. Core Measures). Demonstrates commitment to understand both success and failure to establish best practices. Learns lessons to improve the patient experience. Is responsible and accountable and reports unsafe practices. Makes changes in clinical practice based on quality outcomes and the patient population.COMPLIANCE WITH MEDICATION & PATIENT SAFETY STANDARDS (INCLUDING NATIONAL PATIENT SAFETY GOALS):Meets safety standards. Provides for patient safety according to professional and regulatory standards. Writes legibly.Minimizes override of medications. Identifies and documents allergies, lactation status, height and weight on all patients.Restricts use of unapproved abbreviations. Documents readback of verbal orders, phone orders and critical test results.Double checks medications when required. Performs essential elements of Medication Reconciliation process. Properly identifies patients using two identifiers. Demonstrates safe hand-off communication using SBAR.
