Qualifications

• One year of nursing experience required.
• Current RN license specific to the assigned state.
• Valid CPR certification necessary.
• Valid driver's license and reliable transportation required.
• Liability insurance is mandatory.

Responsibilities

• Perform patient assessments and collaborate with the care team to craft care plans.
• Make referrals to relevant hospice disciplines or document reasons for not doing so.
• Develop and maintain accurate patient care progress records in a timely manner.
• Document changes in pain/symptom management outcomes as appropriate.
• Educate patients, families, and caregivers on appropriate care techniques.
• Maintain regular communication with the clinical manager and care team.
• Execute on-call responsibilities and services as assigned.
• Undertake other duties assigned by the supervisor.
