Hospice Nurse (Camden/Atlantic County/Gloucester County) Full TimeSalary: $80,808 - $93,512 annually based on years of experience + $5,000 Sign-On BonusA compassionate and dedicated job-seeker is needed for the Hospice Nurse opportunity with our company! As a Hospice Nurse, you will be responsible for overseeing the quality and expert care of your assigned hospice patients.
What You'll Do:- Perform patient assessments for comfort and relief of symptoms.
- Establish and implement an individualized plan of care for each patient.
- Assess the need for patient and family education and provide education as needed.
- Obtain appropriate physician orders to achieve symptom management.
- Triage and arrange home visit schedule, demonstrating efficiency in time and mile management.
- Make referrals to additional IDT members and collaborate with social workers related to their involvement.
What You'll Bring to Our Team:- Minimum of two years of nursing experience.
- NJ RN license required; BSN preferred
- Bilingual is a plus!
