Job Summary and Responsibilities

As a Registered Nurse (RN), you will be a pivotal healthcare professional, delivering compassionate, high-quality care that truly impacts our patients' well-being and recovery.Every day, you will leverage your expertise to provide individualized, comprehensive care, making critical assessments, performing skilled procedures, and meticulously implementing patient care plans.   • Performs routine physical/psychosocial assessment to collect basic data: follows the written plan of care, standards, practice guidelines, utilizes pathways, which includes notifying physicians as appropriate
  • Formulates a plan of care for individual needs, including patient population needs. Participates in the planning of routine transitional health care needs (i.e. treatment options, patient placement options)
  • Adapts planned educational programs and information to individual patients and family by modifying teaching strategies or content. Integrates education during the delivery of patient care
  • Ensures efficiency in the delivery of care to determine immediate priorities and to offer solutions for problems. Makes independent clinical decisions based on knowledge drawn from education and experience
  • Follows unit specific safety regulations and guidelines
  • Actively advocates for patient rights and identifies potential conflict. Seeks counsel for resolution of conflict

Job Requirements

Minimum Qualifications:

 

**This position will serve Shasta and Tehama Counties.Travel is required to Tehama County.**

  • HPNA Certification or hospice RN experience within the last 2 years. (May consider training if no qualified candidates apply and operational conditions exist to support training)
  • Associates in Nursing degree from an accredited school of nursing required
  • Current California RN license required
  • Verbal and written communication skills and computer skills required
  • Current California drivers license/automobile insurance required
  • Current BLS certification required

 

Preferred Qualifications:

  • Bachelors of Science in Nursing degree from an accredited school of nursing preferred
  • Completion of ELNEC training preferred
  • Hospice and Palliative Care Nurse certification preferred
