RN Admissions Nurse - Winter Haven, FLCompassionate Care Hospice (CCH), an Amedisys Company
**This position is based in Lakeland, FL and requires daily travel to Winter Haven assigned area. Schedule:
Monday-Friday, 9:00 AM - 6:00 PM.Salary Range:$82,000-$86,000 annually, plus bonus opportunities. Final compensation may vary based on experience and skills.

Responsibilities
- Coordinates admissions of hospice patients by making skilled assessments of patients initial physical, psychosocial and spiritual needs.
- Coordinates the establishment of the initial plan of care with the attending physician, medical director and the interdisciplinary team.
- Provides skilled interventions and instructions to patient and caregivers to provide optimum symptom control and patient comfort under the direction of physician.
- Functions as a liaison between disciplines and facilities during the admission process.
- Develops, prepares and maintains individualized patient care progress records with accuracy, timeliness and according to policies. Submits accurate documentation per care center policy. Records pain/symptom management changes/outcomes as appropriate.
- Communicates significant findings, problems, changes in condition or environment to the clinical manager/physician and other members of the hospice care team. Communicates with the physician regarding the patients' needs and reports changes in the patient's condition; obtains physician orders as required.
- Performs on-call responsibilities and on-call services to patients/families as assigned.
- Performs other duties as assigned.
Qualifications
Required:- Hospice or home health experience
- Current RN license in the state(s) where you will work
- Minimum 1 year of clinical RN experience in an acute healthcare setting
- Current CPR certification
Preferred:- Strong communication and interpersonal skills
- Ability to work independently and manage multiple priorities
- Comfortable with daily travel between facilities
- Proficient in electronic medical record (EMR) systems
