Responsibilities

  • Conduct medical, mental health, and safety assessments for individuals experiencing homelessness across NYC
  • Provide direct, hands-on medical care in both field and office-based settings
  • Identify individuals in crisis requiring immediate medical or psychiatric intervention
  • Assess risk of harm to self or others and initiate emergency interventions according to Mental Hygiene Law Section 9.58
  • Partner with DHS Crisis Coordinators, outreach teams, and community partners to build trust and encourage engagement with services and shelter
  • Coordinate with hospitals, law enforcement, and transport teams to ensure safe transfers and continuity of care
  • Collaborate with hospital clinical teams to support treatment planning and follow-up care
  • Maintain accurate, timely clinical documentation in compliance with program and regulatory requirements

WCS Healthcare is partnering with a Social Service Organization who is seeking compassionate, experienced Registered Nurses to join its Crisis Intervention Team. In this dynamic, field-based role, you will provide critical medical and mental health support to individuals experiencing homelessness across New York City, responding to crises where care is needed most.

Job Function:
  • Conduct medical, mental health, and safety assessments for individuals experiencing homelessness across NYC
  • Provide direct, hands-on medical care in both field and office-based settings
  • Identify individuals in crisis requiring immediate medical or psychiatric intervention
  • Assess risk of harm to self or others and initiate emergency interventions in accordance with Mental Hygiene Law Section 9.58
  • Partner with DHS Crisis Coordinators, outreach teams, and community partners to build trust and encourage engagement with services and shelter
  • Coordinate with hospitals, law enforcement, and transport teams to ensure safe transfers and continuity of care
  • Collaborate with hospital clinical teams, including C.P.E.P., to support treatment planning and follow-up care
  • Maintain accurate, timely clinical documentation in compliance with program and regulatory requirements
  • Maintain authorization to operate as part of a mobile crisis outreach team (DOHMH approval required)
