Qualifications

• Degree in nursing, therapy, or related clinical field with a current RN license.
• MDS experience is required.
• In-depth knowledge of healthcare reimbursement and utilization management.
• Proven experience in strategic leadership and team management.
• Ability to devise and execute both short and long-term strategic plans.
• Excellent verbal, written, and interpersonal communication skills.
• Familiarity with PDPM requirements and regulatory standards.

Responsibilities

• Facilitate daily care coordination meetings to align resources with patient needs.
• Lead Engage/72 HR Admission meetings to enhance care management.
• Manage the integration of restorative nursing with activity programs.
• Communicate timely updates to patients and families regarding conditions and discharge.
• Collaborate with clinical teams to create and implement care plans.
• Coordinate all discharge planning and follow-up communications.
• Monitor quality measures in conjunction with the care coordination team.
• Ensure accurate care documentation according to Medicare and Managed Care guidelines.
• Review Additional Documentation Requests to confirm compliance.
