Job Summary: Under the supervision of the Clinic Manager, assists with managing the
flow of clinic by leading all staff in provision of medical and nursing care, ensuring
high quality standards. Coordinates services with primary care team, acts as primary
liaison. Triages, and assess participants as needed. Acts as gatekeeper for daily clinic
scheduling. Provides skilled nurse care to participants, impalements orders, develops
appropriate plans of care, provides participant, caregiver family and staffing teaching
and communicates participant's condition to caregivers. primary care and the
interdisciplinary team. To ensure continuity of operations assumes the Clinic Manager's
responsibilities in her/his absence.
Qualifications and Requirements:- Education:
- Graduate of a school of professional nursing program required. BSN strongly preferred.
- Current California Registered Nurses License and CPR certification required.
- Shall be at least 18 years of age.
- Experience:
- A minimum of 1 years experience working with a frail or elderly population required.
- Two years of nurse management experience in a hospital, nursing home or community-based setting preferred.
- Shall have either training or related experience in the job assigned
Duties and Responsibilities:- Shall attend in-service education program, including orientation, skill training and continuing education.
- Shall receive in-service training in first aid and in cardiopulmonary resuscitation within the first six months of employment.
- As needed conducts an in-person comprehensive initial nursing assessment with new PACE enrollees. Coordinates with the interdisciplinary team to develop a comprehensive care plan for each participant.
- As needed conducts an in person nursing reassessment semiannually and as needed.
- Functions as a member of the interdisciplinary team. Maintains regular attendance at, and participants in interdisciplinary team meetings to ensure that we are meeting regulatory standards; communicates participant changes, collaborates on care planning decisions and coordination for 24 hour care delivery.
- Provides orientation, initial and annual competencies, supervises and evaluates performance of Clinic RN's, LVN.'s, CNA's, and MA's. Has indirect supervision of personal care provided by Day Center CNA's and PCAs.
- Ensures that the job responsibilities, authorities, and accountability of direct reports are defined and understood. Ensures that clinic staff work with in their scope of practice.
- Engages in team building. Coaches and mentors staff. Instructs and teaches staff as needed. Trains clinic staff to function independently.
- Evaluates the overall effectiveness of the clinic, implementing change and quality improvement as needed in collaboration with Clinic Manager.
- Manages flow of clinic, oversees appointment bookings, triaging and gate keeping in conjunction with primary care and staff. Ensures timely and appropriate care of participants.
- Takes lead in effectively communicating with interdisciplinary team community agencies at meetings and through out the day.
- Oversees and ensures signing off orders, ordering medications from pharmacy and disseminating orders and instructions to appropriate personnel. Ensures implementation of physician orders.
- Coordinates participant care with outside contracted service providers-Home Health Agency staff, nursing homes, assisted living, lab, oxygen, etc.
- Communicates with after-hour staff and any contracted services regarding clinical participant care, to include reviewing participant care documentation. Responsible for following up and communicating with contracted services and the Interdisciplinary Team, as necessary.
- Manages and reviews participant's records, charts, pertinent information and staff's documentation. Documents as needed in medical records according to St. Paul's PACE standards. Minimally provision of Quarterly narrative nursing note, unless a participant's condition requires a more frequent note, indicating participant's progress toward achieving health goals.
- Performs general skilled nursing care to participants. Performs or supervises the administration of prescribed medications and treatments in accordance with nursing standards. Assist in treatment, examination, and testing of participants. Records all medications and treatments administered.
- Responds to and performs triage of incoming telephone calls.
- Oversees scheduling of outside appointments for participants when necessary.
- Assists with updating hospital admissions and calling report to unit. Ensures that participants status change is updated in pharmacy system.
- Notification of the physician of any marked change in the participant's condition.
- Educates participants, family and caregivers in regards to care, medications, and treatments.
- Educates Staff in regards to care and treatment for the elderly as needed.
- Follows all St. Paul's PACE Policies and Procedures, and OSHA safety guidelines.
- Protects privacy and maintains confidentiality of all company procedures, results and information about employees, participants and families.
- Practices Universal precautions, and follows appropriate Infection Control procedures.
- Participates in continuing education classes and any required staff and training meetings. Maintains professional affiliations and any required certifications.
- Participates in and supports Quality Improvement initiatives.
- To ensure continuity of operations assumes the Center Directors responsibilities in her/his absence
- Performs other duties as required or requested.
Physical Demands and Working Conditions:Physical Requirements- Shall be in good mental and physical health, and capable of performing assigned tasks. Requires full range of body motion including handling and lifting patients, manual and finger dexterity and eye-hand coordination. Requires standing, walking, pushing, bending, kneeling and reaching at arm's length and overhead in clinic, rehab department and in community for prolonged periods of time. Must be able to transfer participants needing maximum assist of one. Requires the ability to lift/carry up to 50 pounds and push or pull objects exceeding 150 pounds, using appropriate body mechanics.
Visual, Hearing and Communication Requirements - Requires corrected vision and hearing to normal range, with or without reasonable accommodation. Must be able to document care provided in participants' service records. Must be able to communicate verbally with participants their treatment plan, team members and participants caregivers.
Pressure Factor - Requires working under stressful conditions. Working conditions may be noisy and crowded and fluctuating indoor temperatures. Moderate pressure to meet scheduled appointments while dealing with frail and confused participants. Subject to participants that may have the potential for verbal or physical aggression.
Environmental Conditions - May be exposed to a risk of bodily injury through contact with moving instrumentation, toxic substances, medicinal preparations, bodily fluids, communicable diseases and any other conditions common in a clinic environment and the possibility of exposure to areas of infestation . Subject to unpleasant odors.
