Qualifications

• 1+ year of recent case management or discharge planning experience preferred
• Active State or Compact RN License
• BLS - AHA certification
• Strong assessment skills
• Strong discharge planning skills
• Strong utilization review skills

Responsibilities

• Coordinate patient care plans and services across the continuum of care
• Work closely with providers to facilitate effective discharge planning
• Engage with social workers and external agencies for patient transitions
• Support utilization management to ensure compliance with payer guidelines
• Ensure timely and effective service transition for patients
