The RN Case Manager will work collaboratively with surgeons and patient/family in developing, implementing and documenting individualized plans of care to ensure continuity, quality and appropriate resource use.
• Plan, develop, assess and evaluate care to patients.
• Recommend alternative levels of care.
• Develop and coordinate discharge plan to meet patients identified needs.
• Review, monitor, evaluate and coordinate patient's hospital stay to assure that all appropriate and essential services are delivered timely and efficiently.
• Coordinate and provide continuity of care, including utilization management, transfer coordination, discharge planning, and obtaining authorizations/approvals as needed for outside services for patients/families.
• Conduct daily clinical reviews for quality management activities based on guidelines/standards for patients in a variety of settings, including outpatient, emergency room, inpatient facilities.
• Report any incident of unusual occurrences related to quality, risk and/or patient safety which are identified during case review or other activities.
Required:
• 2+ years combined RN and case management experience
• RN License (California)
• BLS
• Working knowledge of regulatory requirements and accreditation standards
• Effective written and verbal communication, interpersonal, critical thinking and problem-solving skills
• Ability in planning, organizing, and negotiating
• Plus: craniofacial exp
